It's done at random, and there's been quite a bit of research on the matter if you know where to look. [1]

btw the latex for this equation is
blob = \left(\frac{1}{tracking} * \frac{transverse}{range}*\frac{sig_{resolution}}{sig_{radius}}\right)^{2} + \left(\frac{max(0,range - optimal)}{falloff} \right )^{2} \\ \\ Hit Chance = 0.5^{blob}
